FMLA and Leave of Absence (LOA) Information

Advanced notice to Human Resources is required for any family or medical-related absence of more than five consecutive days or after any period of in-patient hospitalization.  The process for requesting a leave of absence is as follows:

  1. Click on the following links to review the Collin College Board Policy regarding Leave and Absence (DEC Local) and the Family Medical Leave Act (FMLA) Fact Sheet #28.

  2. Complete the required documentation specific to your leave circumstance below.

Employee's Own Serious Health Condition

Family Member's Serious Health Condition

Qualifying Military Exigency Leave

Illness/Injury of a Covered Service Member

 

Optional Documentation


If your leave of absence will extend beyond your eligibility to receive earned paid leave and is for your own serious health condition, please complete the forms listed below near the time your paid leave will expire.

Sick Leave Pool


Leave Balance Priority: All eligible paid leave must be used before leave without pay is permitted.

Leave of Absence (Medical): Credit vacation may not be used during a leave of absence for medical reasons

Restricted Leave Status: Employees may placed in a restricted leave status for excessive absenteeism or a pattern of absences or abuse of leave. The restricted leave status would result in restricted use of leave balances, requirements for additional documentation, and would be grounds for leave without pay or other disciplinary action up to and including termination of employment.
